Be a Member of Your Care Team

Your health is your greatest asset. You can help protect it by being an active member of your health care team. Improving safety and quality in health care is not only the business of doctors, nurses or other health care professionals. You, and only you, can be the biggest advocate for your own health.

As a Medicare QIO, we partner with hospitals, doctors’ offices, nursing homes, home health agencies and communities to achieve the goals of better health care for patients, better health for communities, and lower health care costs for everyone.

Our current projects focus on:

  • Reducing hospital acquired infections
  • Reducing pressure ulcers (commonly called bed sores) and the use of physical restraints in hospitals
  • Coordinating care as patients transition from the hospital to other care settings (such as nursing homes, or to their own homes).
