- The CMS Health Disparities Pulse Center - created to inform, educate and share information with the community of stakeholders as it relates to what CMS is doing to track, measure and reduce health disparities for underserved populations, now called priority populations, in the Medicare program.
- The Medicare Quality Improvement Community (MedQIC) Web site offers more than two dozen tools for health disparities, including African-American and Hispanic versions of a mammogram poster. MedQIC is part of QualityNet. Go to the gray MedQIC tab at the top of page. Then look for Health Disparities in the gray menu beneath the tabs. The blue box at the top of the page offers tools, stories, links, other resources and presentations.
- For consumers interested in long term care, The West Virginia Health Care Association’s Consumer Guide to Extended Care Facilities is a collection of useful information designed to assist families select between various long term care options. The document is available for download; print copies are also available by calling the West Virginia Health Care Association at 1-888-298-9842 or AARP’s West Virginia State Office at (304) 344-4740.
- For health care providers, The Health Resources and Services Administration (HRSA), part of the U.S. Department of Health and Human Services, offers a centralized portal for communication as well as a forum for sharing the challenges, successes, tools of the trade and lessons learned as a result of quality improvements efforts. A quick search of this knowledge gateway provides numerous health literacy publications, including the Quick Guide to Health Literacy and Health Literacy by the Numbers.
- The U.S. Department of Labor has re-named and re-launched www.disability.gov, which offers comprehensive information about programs and services to better serve more than 50 million Americans with disabilities, their family members, veterans, employers, educators, caregivers and anyone interested in disability-related information.
